Initial Comments:Based on the findings of an off-site follow-up survey completed on June 23, 2023, OSPTA Home Care and Hospice, was found to have corrected the deficiencies cited under the requirements of 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of an unannounced on-site Medicare Re-Certification and re-licensure survey completed on April 7, 2023.
